mmap.2: MAP_FIXED is no longer discouraged
...
MAP_FIXED has been widely used for a very long time, yet the man
page still claims that "the use of this option is discouraged".
The documentation assumes that "less portable" == "must be discouraged".
Instead of discouraging something that is so useful and widely used,
change the documentation to explain its limitations better.

Starting with Linux kernel 4.5 and 84638335900f1995
(mm: rework virtual memory accounting) RLIMIT_DATA affects
mmap(2). With d977d56ce5b3e88 (mm: warn about VmData over
RLIMIT_DATA) only warnings are emmitted when going over the
specified RLIMIT_DATA in 4.5.
As of 4.7 and f4fcd55841fc9 (mm: enable RLIMIT_DATA by default
with workaround for valgrind) going over RLIMIT_DATA through
mmap(2) is forbidden.

mmap.2: Describe treatment of 'offset' for MAP_ANONYMOUS

The manpage claimed that the offset argument is ignored, and when
I interpreted that as "I don't need to set that register when
doing the syscall", I got failures. I was able to spot two reasons
for that:
What I probably ran into:
At least on x86-64, sys_mmap (in arch/x86/kernel/sys_x86_64.c)
always checks that the offset is page-aligned, even for
MAP_ANONYMOUS.
Another one, could probably trigger on 32-bit x86:
In do_mmap(), there is a check to ensure that pgoff together with
the allocation length won't cause an overflow, even for
MAP_ANONYMOUS.
Document that userspace should pass in zero, since that's
probably what everyone is doing already. (It would also be
possible to describe the constraints on the offset more
carefully, but zero works, and nobody should need to pass in
anything else.)

mmap.2: Clarify MAP_LOCKED semantics

MAP_LOCKED had a subtly different semantic from mmap(2)+mlock(2)
since it has been introduced.
mlock(2) fails if the memory range cannot get populated to
guarantee that no future major faults will happen on the range.
mmap(MAP_LOCKED) on the other hand silently succeeds even if
the range was populated only partially.
Fixing this subtle difference in the kernel is rather awkward
because the memory population happens after mm locks have been
dropped and so the cleanup before returning failure (munlock)
could operate on something else than the originally mapped area.
E.g. speculative userspace page fault handler catching SEGV and
doing mmap(fault_addr, MAP_FIXED|MAP_LOCKED) might discard portion
of a racing mmap and lead to lost data. Although it is not clear
whether such a usage would be valid, mmap page doesn't explicitly
describe requirements for threaded applications so we cannot
exclude this possibility.
This patch makes the semantic of MAP_LOCKED explicit and suggests
using mmap + mlock as the only way to guarantee no later major
page faults.

mmap.2: Add note about partial page in BUGS section

This adds a note about Linux behavior with partial page at the end
of the object. The problem here is that a page that contains only
part of a file (because the file size is not multiple of PAGE_SIZE)
stays in page cache even after the mapping is unmapped and the file
is closed. So if some process dirties such page, other mappings
will see the changes rather than zeroes.
I've also attached a reproducer which is a stripped down version of
the LTP test. The child creates a file of the size of PAGE_SIZE/2,
maps it, changes the content after the PAGE_SIZE/2. The parent
waits for the child to exit, maps the same file, and checks the
content after PAGE_SIZE/2. Uncommenting the msync() makes the test
succeed.
